At 360 Supports LLC, our In-Home Support services help individuals with developmental and intellectual disabilities live successfully in their own home and community through person-centered, skill-building, routine, and advocacy supports based on the Individual Support Plan (ISP). Supports may include Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) such as hygiene, dressing, meal preparation, light housekeeping, laundry, medication reminders (as permitted), scheduling, and building daily living routines—always promoting independence and choice. We also provide community access and safety supports, including planning outings, using transportation safely, practicing social skills, and participating in meaningful activities that strengthen community integration. In addition, staff provide advocacy support by helping individuals communicate their preferences, understand their rights, and participate in decisions about their services, routines, and goals—while coordinating with the support team as appropriate to ensure needs are addressed. Services are delivered with dignity and respect, documented according to the plan for supports, and coordinated with the individual’s team to ensure goals are maintained and health and safety needs are met.
