In-Home Support Services (IHSS)

For five years, our In-Home Support program has helped individuals remain successfully in their own homes while developing the daily living skills necessary for long-term independence.

Our IHSS supports include:

  • Daily routine and independence training
  • Medication reminders and monitoring
  • Safety monitoring and risk mitigation
  • ISP-driven skill development

Community Coaching & Engagement

Through this service, we help individuals:

  • Develop natural supports
  • Increase confidence in public settings
  • Explore meaningful day and pre-vocational opportunities

Over the past five years, this program has helped many individuals move from limited engagement to active community participation, consistent with HCBS integration requirements.

Mental Health Outpatient Services

Our outpatient mental health supports focus on strengthening emotional wellness and functional stability. Services may be provided with or without medication management.

Services include:

  • Individual Therapy (Psychotherapy)
    One-on-one counseling to address depression, anxiety, trauma, adjustment issues, and other mental health concerns.
  • Family Therapy
    Sessions are designed to improve communication, strengthen relationships, and enhance support systems.
  • Group Therapy
    Structured therapeutic groups focused on skill-building, coping strategies, and peer support.
  • Mental Health Skill-Building (MHSS) (DBHDS community service)
    Helps individuals develop independent living skills and coping strategies.
  • Psychosocial Rehabilitation (PSR) / PRP
    Focuses on restoring daily functioning, improving social skills, and promoting community integration.
  • Care Coordination and Case Management (if authorized)
    Connecting individuals with community resources and additional support services.

Sponsored Residential Services

For individuals who require 24/7 support in a family-style setting, our Sponsored Residential program provides a safe, stable, and nurturing home environment.

For five years, 360 Supports LLC has maintained strong oversight systems to ensure every sponsored placement meets DBHDS health, safety, and quality standards.

Our program provides:

  • Qualified, trained sponsor homes
  • Individualized daily supports
  • Medication administration (as authorized for Sponsored Residential and Group Home settings)
  • Monthly face-to-face provider visits
  • Quarterly quality reviews
  • Incident management and reporting
  • ISP implementation monitoring

Group Home Services (DBHDS/HCBS)

360 Supports LLC provides a safe, home-like group living environment that supports individuals with disabilities in living with dignity, independence, and choice.

Our services are person-centered and aligned with each individual’s Individual Support Plan (ISP). Trained staff provide daily living assistance, community engagement support, safety oversight, and consistent documentation to meet DBHDS and HCBS standards.
